For pronounced jowls and a slack chin-neck area, a facelift is absolutely the most effective treatment. While moderate wrinkles and less pronounced jowls or “hamster cheeks” can be treated with hyaluronic acid or fat transfer and a sagging chin-neck area can be improved by targeted liposuction, a surgical face lift is advisable in cases in which a significant skin surplus is involved.
In recent years, the surgical techniques have improved tremendously. During a normal facelift, not only the surface skin is tightened, but also the layers of connective tissue and muscle underneath, the so-called SMAS. Depending on whether the facelift is to lift the cheek area or smooth the chin-neck area, we can individually vary the direction of tightening during surgery according to our previous consultation with the patient.
It is extremely important to us that the result appears natural and the delicate scars remain as unnoticeable as possible. The fine incision usually begins at the hairline in front of the ear, runs behind the so-called tragus (the small flap of cartilage that sticks out directly in front of the ear canal), goes around the earlobe and remains hidden behind the ear conch before returning to the hairline far behind the ear. The so-called mini-lift or S-lift and the MACS-lift (Minimal Access Cranial Suspension) leave smaller scars and are performed in front of the ear in the temporal area and only make sense when the aging process is not far advanced.
A facelift can be complemented with an upper and lower eyelid plasty. During the operation, we often perform supplementary liposuction in the neck area or a dermabrasion of the fine wrinkles on the upper lip.
Because a facelift is a very demanding operation that lasts several hours, a good twilight sedation or general anesthesia is advisable. At the end of the operation, a cotton-padded head bandage will need to be worn for 5-7 days to optimally protect the areas that need to heal. Our patients normally remain in our clinic for one night and can be released the following day. After about one week, we remove the bandage and most of the noticeable stitches. Then we wash and blow-dry the hair so that the patient can begin to move about in public.
